To receive a payment from the PNI settlement fund, you must submit a completed claim by 11:59 p.m. (PT) on December 14, 2017.
It is recommended that you scan, import, and gather all supporting documentation (receipts, bank statements, card statements, etc.) before getting started, as you will need to upload these documents in support of your claim. You cannot save your progress as you proceed through the module.
Additionally, you may wish to review the Settlement Notice and Sections 2.1 through 2.2 of the Settlement Agreement to understand who is eligible for a payment and the nature of the expenses or losses that can be claimed.
This online session will expire after 30 minutes of inactivity, and you will need to begin again if your claim is not completed.
Please be advised that you may submit claims for ten (10) affected cards at one time. If you have additional affected cards, you must include them in a separate claim.
